Submitter/Receiver ID Application

The Submitter/Receiver ID Application is used when an Organization needs to obtain a Submitter or Receiver ID in order to exchange data with the Customer Service Front End System (CSFES). This form must be completed and returned to CSSC Operations before an ID can be issued.

Important note:  The Data Exchange Mechanism is often not entered on the application and causes delays in processing. Please ensure you select the mechanism your organization uses to exchange data with the CSFES.
